Invisalign is an orthodontic system that uses a numbered series of custom clear aligners, each one shaped slightly differently from the last, to walk your teeth into position a fraction of a millimetre at a time. The aligners are made from a multilayer thermoplastic called SmartTrack, which holds a gentler and more constant force than the stiff plastic used in older aligner systems.

The reason it took off here is fairly simple. A lot of our patients are adults in client-facing jobs, teachers, cabin crew, doctors, brides with a date circled on the calendar. They want their teeth fixed without a mouthful of metal showing in every meeting and every photo. Clear aligners are close to invisible at conversation distance, and they come out for the wedding, the presentation, or the flight.

Why patients pick aligners over fixed braces

Three things come up in almost every consultation we run, and they are usually in this order.

Appearance. No brackets, no wires, no elastic ties that stain with coffee or karak.
Comfort. Smooth BPA-free plastic instead of edges that catch the inside of your cheek. There is pressure for a day or two after each new aligner, but no wire poking anything.

Cases where Invisalign is not the right answer

Clear aligners handle crowding, spacing, mild to moderate overbite, deep bite, open bite and rotations very predictably. They struggle more with severely rotated canines and premolars, teeth that need to be moved bodily across an extraction gap, and skeletal bite problems where the jaws themselves are mismatched rather than the teeth. Those cases may need fixed appliances, temporary anchorage devices, or a combined surgical plan. How does Invisalign actually move your teeth?

Each aligner is manufactured to the shape your teeth should be in at that stage, not the shape they are in now, and the mismatch between the two is what creates the force. That gap is deliberately tiny, usually about 0.25 mm of movement per aligner, because bone remodels slowly. Push harder and the tooth does not move faster; the ligament around the root gets compressed, blood supply drops, and movement actually stalls.

Plain plastic cannot grip a smooth, rounded tooth well enough to rotate or upright it. That is why we bond small tooth-coloured composite bumps, called attachments, onto certain teeth. Stage 1: consultation, records and the iTero scan

Your first appointment takes about 45 minutes. The orthodontist checks the health of your gums and existing fillings first, because you cannot move teeth in an inflamed mouth or through untreated decay. Then we take photographs, an OPG or CBCT radiograph, and an iTero digital scan that builds a 3D model of both arches in a few minutes. No impression trays, no gagging. The scanner also produces a rough simulation of your projected result on screen, which is useful for expectation setting, though it is a preview and not a promise.

Stage 2: the digital treatment plan

Your scan goes into planning software where the orthodontist sequences every movement, tooth by tooth, stage by stage. Refinements are extra aligners ordered near the end of treatment when a few teeth have not tracked exactly as planned. They are normal, not a failure, and they happen in a large share of cases. We rescan, order a short additional series, and finish the detail. Ask during your consultation whether refinements are included in the quoted fee, because that single question can be worth thousands of dirhams.

Stage 5: retention

Teeth drift back. That is biology, not bad treatment, and it is strongest in the first year after the aligners come out. We usually bond a thin fixed wire behind the lower front teeth and provide a clear removable retainer for night wear, indefinitely. What daily life with aligners is really like

Life with aligners is easy once the routine is built, and the whole routine comes down to three habits. Take them out for everything except water. Brush before you put them back in. Keep the case with you, because more aligners die in restaurant bins than anywhere else on earth.

Clean the trays with lukewarm water and a soft brush, never hot water, which distorts the plastic permanently. Skip whitening toothpaste on them, since the abrasives cloud the surface. Does Invisalign hurt?

Invisalign causes pressure and tenderness rather than pain, mostly in the first 48 hours after each new aligner. Patients describe it as a tight, bruised feeling when biting. Can people tell I am wearing Invisalign?
